Grant Reporting

Report Preparation Instructions
Please use the following steps to prepare and submit grant reports:

  • Log in to our grants management portal with your email address and password.
  • Once logged in, you will be on the Applicant Dashboard.
  • View your open grant with the Foundation under the “Active Requests” tab.
  • View the annual report listed as a “Follow-up Form”, and click the blue “Edit” button to begin preparing the report.
  • As you prepare the annual report, you can access the annual financial report template here (MSWord) (PDF)

For those who are unsure if they have an account on our online grants management portal, please contact Mt. Sinai staff at to receive your username. If your organization has received grant funding from Mt. Sinai in the past, it is likely that you already have an account in our system. Please do not create a new account.
